Holistic Care’s Innovative Community Participation Program is designed to help NDIS participants develop the skills needed to actively engage with their community. The program features highly skilled Community Engagement Practitioners who work alongside participants to expand their opportunities for community involvement and employment. Adopting a capacity-building, community-based approach, these practitioners collaborate with participants in their local areas to enhance the likelihood of a successful transition to informal supports and/or employment.

The focus of Community Engagement Practitioners is on resilience, well-being, and personal growth, ensuring each service is tailored to meet the participant’s unique goals and needs. Through this program, participants can expect support in:

• Understanding the role of community engagement, social contribution, and relationships in maintaining personal well-being
• Developing self-awareness and self-care practices
• Identifying personal interests, skills, and passions and connecting them to employment opportunities
• Enhancing emotional awareness and self-regulation
• Improving adaptive social skills and understanding social norms and expectations
• Exploring their relationship with their disability, fostering independence, confidence, and self-worth
• Navigating systems and programs to effectively participate in community and employment pathways
• Preparing for and accessing training and employment opportunities
• Building the capacity to seek community assistance independently, without ongoing formal supports
