Our Support Coordination services include:
– Explaining your NDIS plan and budget categories
– Linking you to suitable service providers (nursing, transport, therapies, etc.)
– Building your capacity to manage your own supports over time
– Monitoring service delivery and resolving issues with providers
– Preparing reports and progress updates for the NDIA
– Providing crisis support and troubleshooting complex situations
For participants with more complex or high-risk situations, our Specialist Support Coordinators (Level 3) offer advanced coordination and case management. We work closely with allied health, clinical, and behavioural professionals to ensure continuity of care and safety.
