What is a wound care nursing assessment?
A wound care nursing assessment is completed by a Registered Nurse. The nurse checks the wound, surrounding skin, dressing needs, pain, infection risks and support requirements. A wound management plan is then prepared for safe and consistent care at home.
Can the NDIS fund wound care and dressing support?
The NDIS may fund wound care support when it relates to a participant’s disability support needs and meets the NDIS funding criteria. We can review your support needs and explain what information may be required.
Do you provide pressure care support?
Yes. We can support pressure care routines, including repositioning, skin checks, pressure injury monitoring and reporting changes to the Registered Nurse or treating clinician.
What types of wounds do you assess?
Our Registered Nurses can assess pressure injuries, surgical wounds, diabetic foot wounds, skin tears, venous leg ulcers, traumatic wounds and other chronic or complex wounds requiring ongoing management.
Can support workers help with dressing changes?
Support workers may assist with approved dressing routines when this is included in the participant’s care plan and they have been trained by a Registered Nurse . Complex or high-risk wounds remain under clinical oversight.
Do I need a referral for a wound care assessment?
A referral may not always be required. We can discuss your circumstances, review your support needs and advise whether information from your GP or treating clinician is needed.
Can your nurses train my support workers?
Yes. Our Registered Nurses can provide practical training and written instructions so support workers understand approved dressing routines, infection prevention, pressure care, documentation and escalation requirements.
